摂食・嚥下障害者の障害の程度は多様であり食事摂取量の改善には個々の患者の摂食能力に応じた食事を提供することが必要である。今回我々は嚥下食の段階化に取り組み嚥下食を食形態の難易度順の5段階 (IV) に設定した。嚥下食IIIIは嚥下訓練のための食事IVは必要栄養量を確保するための食事Vは嚥下食から固形食への移行を目的とした準備期間の食事とした。 嚥下食5段階化の取り組みを評価するため嚥下食喫食者23名を対象に平均喫食率を調査した。嚥下食IV喫食者では7.5±2.1割Vでは7.4±1.9割両者を併せて7.4±2.0割の喫食率であり嚥下食が段階化される前の調査 (日農医誌 57: 83-882008) での平均喫食率より高かった。また症例において5段階化にしたことで経口摂取に十分に移行できた。嚥下食の5段階化は喫食率の評価で支持された傾向が認められており2008年7月から当院において正式に導入されている。
