サイクロスポリンA (CsA)は,自己免疫疾患分野では,ベーチェット病におけるブドウ膜炎,乾癬やI型糖尿病で良好な成績が報告されているが,全身性エリテマトーデス(SLE)の治療薬としては知見が乏しい.今回報告した症例は,両側大腿骨骨頭壊死を合併し,かつ原疾患の再燃をきたしたSLE症例であり,維持量のステロイド剤(以下ス剤)を併用しながら, CsAを3mg/kgから投与した.第40日に6mg/kgに増量し,ほぼ同時に補体価の正常化傾向がみられた.血中BMGの上昇が出現したため4mg/kgに減量した後も,補体価の正常化傾向が持続,かつ皮疹,関節痛も消褪した.さらに,リンパ球数の増加,抗Sm抗体,抗RNP抗体力価の減少を認めたが, suppressor inducer細胞比率,血清免疫グロブリン値は不変であった. CsAは,その副作用の発現のため,ス剤大量投与が困難なSLE症例においては,寛解導入効果を期待できる薬剤である.
