慢性関節リウマチ(RA)患者11例に対し,血漿交換療法(PP)を施行し,臨床所見の変化を観察するとともに血中免疫複合体(CIC)を原理の異なる3法で測定し, PPのモニタリングとしての有用性を検討した. PPは二重膜濾過法にて行い, 1回の血漿処理量を2,000mlとし,これを週1回,計4回施行した. CICはRaji cell法(RJC),抗C3法(AC 3), C l q固相法(C l q SA)の3法で測定した. AC 3法陽性例とRJC法陽性例でPP施行後4週まで, C l q SA法陽性例で1週まで有意にCICの低下を認めた.また, AC3法陽性例では, PP施行後8週までCICの有意の低下とともに, Lansbury indexの有意の改善が認められた.しかし, RJC法陽性例, C l q SA法陽性例ではCICの低下とLansbury indexの改善に関連性はみられなかった.したがって, RAに対するPPのモニタリングとして3法の中ではAC 3法が最も有用と考えられた.
