概要Amyloidosisは 近 年本邦 で著 しく増 加 し,そ の頻度 は一 部の西 欧諸 国に接 近ない し追 い抜 く位 にな つた.最 近 の本邦例57例 の検 索結 果を報 告 し,併 せ て 自験 例1例 を追 加 した.続 発性amyloidosis の基 礎疾患 と して近 年悪性 腫瘍,リ ウマ チ,慢 性気 管支疾 患 が増 加 しっ ゝあ る.amyloidosisの 臨床 症状 は甚 だ多岐 に亘 るが,本 邦例 では胃腸障 害 が最 も多 く,心障 害,肝 腫大,腎 障害 な どの頻 度 が高 かっ た.amyloidosisの 臨沫 診断 には,生 検 法で得 た材 料 をコン ゴー赤 で染 めて偏光顕 微鏡的観 察 を 行 な うの が最 もす ぐれた方法 であ る,para-amyloidosisと い う用 談 をこは概 念の混 乱が あ る が,paraamyloid物 質 を顕著 に持っ た症 例 と解 したい.そ うい う物質の構 造にっい て一仮説 を呈示 した.症 例 は46才,女 。呼 吸困難 と浮腫 を主訴 と し,大 舌症 があった ガ,う っ血性 心不全の 臨床診 断の も とに死 亡 した.剖 検 に よりpara-amyloid物 質ば か りか ら成 りた った原発性amyloidosisと 判 明 した.
