Thyrotoxicosis is a rare but recognized complication of thyroid fine needle aspiration biopsy (FNAB). Post-aspiration thyrotoxicosis may present with symptoms and result in a range of biochemical abnormalities. A course of steroids may result in prompt resolution of symptoms and progressive improvement in the biochemical derangements. Here we present the case of a 65-year-old woman who was admitted to our hospital with raised thyroid hormone levels (fT4 >100 pmol/L, fT3 17.1 pmol/L) several days after she had FNAB of a mixed thyroid mass. She was noted to have a markedly raised C-reactive protein (CRP) of 237 mg/L with a normal white blood cell (WBC) count of 7.7×10/L. She was also found to have a deranged liver function test with a raised alanine aminotransferase (ALT) of 203 U/L and alkaline phosphatase (ALP) of 259 U/L. On physical examination, she was found to have a resting tachycardia, persistent fever and a large, slightly tender thyroid swelling. Investigations excluded any focus of infection. A nuclear medicine thyroid uptake scan did not show any significant uptake within the thyroid gland or focal activity therefore suggesting thyroiditis. She was treated with a reducing course of prednisolone and experienced prompt resolution of her symptoms and progressive improvement in the biochemical derangements. Thyrotoxicosis can complicate fine needle aspiration biopsy (FNAB) of the thyroid. The exact mechanism is uncertain. Post-aspiration thyrotoxicosis can rarely present with overt symptoms and a range of biochemical derangements. These abnormalities tend to improve as the thyrotoxicosis resolves. The natural clinical course of untreated post-aspiration thyrotoxicosis is not clearly defined. In our case, a reducing course of oral steroids seemed to have hastened recovery.
