Aim. Of the study was to investigate the influence of renal function, measured by the glomerular filtration rate on long-term outcomes of patients with acute myocardial infarction undergoing percutaneous coronary intervention. Material and methods. This retrospective study included 179 patients who were admitted to hospitals north-eastern district of Moscow with acute myocardial infarction (AMI). Patients were divided into two groups: 1) eGFR ≥ 60 mL/ min/1,73m2; 2) eGFR < 59 mL/min/1,73m2. The cutpoint was death and complications during hospitalization and followup. The mean follow-up period was 18 months. Results and discussion. The 2nd group demonstrated a significantly greater comorbidity. Stenosis of the left main coronary artery (LCA) > 50% was an independent risk factor for death (p=0,021). The mortality rate during follow-up in the 2nd group was significantly higher than the 1st group (p=0,001). Undergoing PCI in patients of the 2nd group was a significantly better long-term survival compared with conservative treatment strategy (Log-rank test = 0,002). Conclusion. Tactics of early PCI compared with conservative tactics of AMI patients with low GFR contributes to better survival.
