The number of deaths by traffic accidents annually increases. Charred and mutilated bodies arise as the result of this thermal and traumatic event. Consequently, Forensic Odontology plays an important role enabling the cadaveric examination of the remaining hard tissue. However, dental autopsies essentially require accurate and reliable ante-mortem data for a proper comparative human identification. The objective is to report a case of human identification, involving a victim of traffic accident, hampered by misfiled dental records. In 2014, fatal victims of traffic accident were referred charred for human identification. The first victim was identified by DNA exam; while the second victim enable the procedure for dental identification once ante-mortem dental records were found. Mismatches were detected during the comparative procedure between ante-mortem and post-mortem dental data. However, positive matches were also found, indicating potential misfiled information into the ante-mortem dental records. Based on that, DNA exams were carried for identification. Positive identification was reached, indicating that the ante-mortem dental records were misfiled. Misfiled dental records hamper dental autopsies; delay the identification process; and eventually raise the costs for forensic expertise due to the need for DNA exams. Thus, the present study highlights the relevance of proper ethical behavior in the clinical routine of Dentistry, especially during the registration of technical procedures and storage of dental records.
