INTRODUÇÃO: O uso dos implantes de gel de silicone para o aumento do volume das mamas marcou uma nova era na história da Cirurgia Plástica. A abordagem inicial pelo sulco submamário foi estendida para incisões axilares e na região areolar. A posição do implante com relação ao músculo peitoral também apresentou variações. OBJETIVO: Descrever o uso do descolamento mamário subfascial rombo através de uma abordagem periareolar inferior e transmamária, praticado com um afastador articulado. MÉTODO: Vinte e seis pacientes com hipomastia foram estudadas de janeiro a setembro 2009. Todas foram submetidas a mastoplastia de aumento com prótese de gel de silicone via areolar inferior através de uma abordagem periareolar inferior e transmamária, praticada com um afastador articulado para as gluteoplastias de aumento sem auxílio videoendoscópico. RESULTADOS: A idade média foi de 29,96 anos. Todos os casos apresentaram boa evolução. O tamanho médio das próteses foi de 294,80 ml. Houve deiscência parcial em apenas um caso e total bilateral em outro. Não houve formação de hematomas ou infecção. CONCLUSÃO: O uso do descolamento rombo subfascial mamário, praticado com descolador articulado, por via areolar inferior mostrou ser uma alternativa factível, relativamente rápida, segura e reprodutível para a colocação de implantes de silicone nas mastoplastias de aumento. BACKGROUND: The usage of textured silicone prosthesis in breast augmentation has marked a new era in the history of Plastic Surgery. The initial approach using the inframammary fold was extended to axillary or periareolar incisions. Even the position of the implants has varied concerning the pectoralis major muscle. OBJECTIVE: To describe the use of subfascial periareolar breast augmentation undermining using a breast dissector. METHODS: Twenty six patients with hypommasty were studied from January to September 2009. All of them were submitted to breast augmentation surgery with silicone implant using a subfascial periarealar pathway without endoscopic assistance. RESULTS: The medium age was 29.96 years old. The medium silicone prosthesis volume used was 294.80 ml. The patients post surgical evolution was considered good. We have no infection or hematoma. There was 1 case of partial dehiscence and one case of total and bilateral dehiscence. CONCLUSION: The subfascial periareolar breast augmentation undermining using a breast dissector appears to be an alternative which provides a rapid, safe and reproducible surgical for breast augmentation surgery using silicone implants
