〔目的〕人工膝関節置換術後の自動介助練習と他動練習の練習方法の違いが膝関節可動性と疼痛に及ぼす影響を評価すること。〔対象〕人工膝関節の機種術者術式が同一の関節リウマチ患者34名。〔方法〕被検者を無作為に自動介助群17名と他動群17名に分けて理学療法を提供した。理学療法の時間・回数・量は統一した。測定項目は他動屈曲角度他動伸展角度120度獲得日数CPM日数練習時疼痛とし各々の群間あるいは群内比較を行った。〔結果〕群間比較は他動屈曲角度120度獲得日数CPM日数練習時疼痛で有意差が認められた。群内比較は他動屈曲角度他動伸展角度で有意差が認められた。全測定項目で自動介助群では他動群と比べて良好な改善傾向がみられた。〔結語〕自動介助練習は膝関節の可動性改善と練習時疼痛の軽減により効果的であるが適用者には理学療法士の十分な説明と指導が必要である。可動域の改善と疼痛軽減が効果的に得られる練習方法と各々の練習方法については科学的根拠に裏づけされる必要がある。
