Possibly the most under-diagnosed of all medical conditions are the episodic ones linked with the brain. These paroxysmal disorders are often ignored because when the patient presents to the mental health practitioner (MHP) they appear well. Very often, they deny that anything is wrong, and yet their families cry out for the MHP to help them. But this is only one side. These patients may have one or more of several conditions and many are not even listed in diagnostic bibles such as DSM5. They require revised terminology, which is sometimes non-existent. They need to be diagnosed, yet the criteria to evaluate them are usually unknown. The broader impacts on spouses, partner and families must be understood and in return how much role those varying "ethicospirituobiopsychofamilioethnicosociocultural" considerations may play in alleviating or aggravating these conditions. Episodic phenomena are difficult to evaluate, and the need for appropriate management after the initial and continued evaluations make them a challenge. Additionally, the absence of proper diagnostic listings makes even potential insurance remuneration more difficult. a
