Цель исследования — оценить состояние микроциркуляции, церебрального перфузионного и внутри!черепного давлений у больных с изолированной тяжёлой черепно!мозговой травмой, и определить возможные взаимоотноше! ния между ними. Материал и методы. Проведено 148 исследований у 16 пострадавших с тяжелой черепно!мозго! вой травмой (ТЧМТ). Пострадавшие по исходу травматической болезни головного мозга были разделены на две группы: I — благоприятный исход (8 человек) и II — летальный исход. Исследование микроциркуляции проводили с использованием накожной лазерной допплеровской флоуметрии с помощью лазерного анализатора капиллярно! го кровотока (ЛАКК!01) отечественного производства (НПО «ЛАЗМА», РФ). Всем пострадавшим проводили оперативные вмешательства по удалению эпи!, субдуральных и внутримозговых гематом. Интраоперационно им устанавливали субдуральный/ интрапаренхиматозный датчик внутричерепного давления (ВЧД) «Codman» фирмы «Jonson & Jonson» (Великобритания). Расчёт церебрального перфузионного давления (ЦПД) производили по об! щепринятой формуле: ЦПД=САД!ВЧД. Исследования ВЧД, ЦПД и микроциркуляции проводились в 1!е, 3!и, 5!е и 7!е сутки послеоперационного периода. Регистрацию их показателей проводили одновременно. В группе больных с благоприятным исходом проведено 90 исследований, а в группе с летальным исходом — 58. Результаты. Отсут! ствие корреляции между изменениями САД, ЦПД и показателями микроциркуляции свидетельствовало о том, что величину ВЧД определяли характером церебрального повреждения, что являлось ведущим и определяющим пока! зателем в диагностике и лечении вторичных повреждений головного мозга. Амплитуда низкочастотных колебаний находилась в прямой корреляционной связи с внутричерепным давлением, что указывало на возможность косвен! но оценивать церебральную перфузию и нарушение мозгового кровотока с их помощью у пострадавших с тяжёлой черепно!мозговой травмой. Заключение. Метод лазерной допплеровской флоуметрии позволяет не только качест! венно, но и количественно оценивать изменения в системе тканевого кровотока при тяжелой черепно!мозговой травме. При ней страдают как локальные, так и центральные механизмы ауторегуляции тканевого кровотока с до! минированием последних. Objective: to evaluate the states of microcirculation, cerebral perfusion intracranial pressures in patients with isolat! ed severe brain injury (SBI) and to determine their possible relationships. Subjects and methods. 148 studies were performed in 16 victims with SBI. According to the outcome of brain traumatic disease, the patients were divided into two groups: 1) those who had a good outcome (n=8) and 2) those who had a fatal outcome (n=8). Microcirculation was examined by skin laser Doppler flowmetry using a LAKK!01 capillary blood flow laser analyzer (LAZMA Research!and!Production Association, Russian Federation). All the victims underwent surgical interventions to remove epi!, subdural, and intracerebral hematomas. A Codman subdural/intraparenchymatous intracranial pressure (ICD) sensor (Johnson & Johnson, United Kingdom) was intraoperatively inserted in the victims. Cerebral perfusion pressure (CPP) was calculated using the generally accepted formula: CPP = MBP (mean blood pressure) — ICD. ICD, CPP, and microcirculation were studied on postoperative days 1, 3, 5, and 7. Their values were recorded simul! taneously. Ninety and 58 studies were conducted in the group of patients with good and fatal outcomes, respectively. Results. No correlation between the changes in MBP, ICD, and microcirculatory parameters suggested that the value of ICD was determined by the nature of brain damage and it was the leading and determining indicator in the diagno! sis and treatment of secondary cerebral lesions. The amplitude of low!frequency fluctuations directly correlated with ICD, which indicated that they might be used to evaluate cerebral perfusion and impaired cerebral circulation indi! rectly in victims with severe brain injury. Conclusion. The laser Doppler flowmetric technique makes it possible not only to qualitatively, but also quantitatively determine changes in the tissue blood flow system in severe brain injury. With this technique, both the local and central mechanisms which are responsible for changes in tissue blood flow autoregulation suffer, with a preponderance of the latter mechanisms.
