Цель: исследовать точность измерения внесосудистой воды легких (ВСВЛ) при помощи методов термохромо4 дилюции (ТХД) и изолированной термодилюции (ИТД) на модели пневмонэктомии и последующего вентиля4 тор4индуцированного повреждения легких (ВИПЛ) у овец. Материалы и методы: исследование выполнено на базе научно4исследовательской лаборатории университета г. Тромсе. В эксперимент включено 12 овец ве4 сом 35,6±4,6 кг. В условиях общей анестезии и контролируемой ИВЛ животным была выполнена торакотомия и правосторонняя пневмонэктомия. После измерения показателей системной гемодинамики и ВСВЛ, живот4 ные были разделены на две группы: протективной вентиляции (ПВ, n=6) с дыхательным объемом (ДО) 6 мл/кг и положительным давлением в конце выдоха (ПДКВ) 2 см H2O и группа вентилятор4индуцированного повреж4 дения легких (ВИПЛ, n=6) с ДО 12 мл/кг и ПДКВ 0 см H2O. Для измерения волюметрических показателей и ВСВЛ использовали методы термохромодилюции и термодилюции (соответственно, мониторы Cold Z4021 и PiCCOplus, Pulsion, Германия). Регистрировали показатели легочной гемодинамики, респираторной механики и газового состава крови. После эвтаназии легкие животных забирались для определения контрольного значе4 ния ВСВЛ методом посмертной гравиметрии (ПГ). Результаты: в группе ВИПЛ развивался выраженный аль4 веолярный отек легких, что сопровождалось гипертермией, повышением давления в легочной артерии и ростом внутрилегочного шунтирования. Анализ данных показал наличие тесной корреляции между значениями ВСВЛ, полученными методами ТХД и ИТД с ПГ (соответственно, r=0,95 и r=0,81, p<0,01; n=12). Отклонение от ПГ для ТХД и ИТД составило 0,57±2,05 мл/кг и 2,68±3,61 мл/кг (M±2δ, p<0,05). Заключение: оба исполь4 зованных метода обладают приемлемой точностью и адекватно отражают как снижение ВСВЛ после удаления легкого, так и ее повышение на фоне ВИПЛ. Волюмотравма может быть ключевым фактором, провоцирующим развитие постпневмонэктомического отека легких. Objective: to study the accuracy of pulmonary extravascular water (PEVW) measurement by thermochromodilution (TCD) and isolated thermodilution (ITD) on a model of pneumonectomy and subsequent ventilator4induced lung lesion (VILL) in sheep. Materials and methods: the study was conducted at the Research Laboratory of University of Tromse. The experiment included 12 sheep weighing 35.6±4.6 kg. Thoracotomy and right4sided pneumonectomy were performed in the animals under general anesthesia and controlled artificial ventilation. After measurement of the para4 meters of systemic hemodynamics and PEVW, the animals were divided into 2 groups: 1) 1) those undergoing pro4 tective ventilation (PV, n=6) with a tidal volume (TV) of 6 ml/kg and a positive end4expiratory pressure (PEEP) of 2 cm H2O and 2) those with VILL (n=6) with a TV of 12 ml/kg and a PEEP of 0 cm H2O. TCD and ITD (Cold Z4021 and PiCCOplus monitors, respectively (Pulsion, Germany)) were used to measure volumetric parameters and PEVW. The parameters of pulmonary hemodynamics, respiratory mechanics, and blood gas composition were recorded. After euthanasia of the animals, their lungs were taken to determine the control value of PEVW by postmortem gravimetry (PG). Results: in the VILL group, significant pulmonary alveolar edema developed, which was followed by hyper4 thermia, elevated pulmonary pressure, and increased intrapulmonary shunting. Analysis of the data indicated a close correlation between the PEVW values obtained by TCD and ITD with PG (r=0.95 and r=0.81, respectively; p<0.01; n=12). The PG deviation was 0.57±2.05 and 2.68±3.61 ml/kg for TCD and ITD, respectively (M±2δ, p<0.05). Conclusion: both used techniques have an acceptable accuracy and adequately reflect both a reduction in PEVW after pneumectomy and its increase in the presence of VILL. Volumotrauma may be the key factor that provokes the devel4 opment of postpneumectomic edema of the lung.
