References Transition is defined as the “planned purposeful movement of the adolescent from child-centered to adult orientated care”. It has been shown to be a difficult time for adolescents with Type 1 diabetes (T1DM) leading to poor long-term outcomes including decreased clinic attendance, poor glycaemic control and increased diabetes related hospital admissions (DRHAs). The implementation of transition programmes like “ReadySteadyGo” at University Hospital Southampton (UHS) aim to improve care and consequently long-term outcomes for patients. The main aim of this study was to determine whether the implementation of a structured transition programme according to national recommendations could improve long term outcomes for young people with T1DM.
