Female adolescents are susceptible to have nutritional problems which may be resulted from strict diet, life style, body image, physical activities, and knowledge about balanced diet. This study was purposed to analyze the correlation between body image, physical activities, and nutritional status of female adolescents in the eighth grade of SMPN 20 Surabaya (state junior high school). This cross sectional analytic study involved 78 female students as the subject of study. The instruments used in this study were Body Shape Questionnaire (BSQ), PAL (Physical Activity Level), and antropometric measurement (body weight and height).The results of this study showed that nearly half of the respondents had positive body image totaling 30 students (38.5%); most of them totaling 59 students (75.6%) did light physical activities; and most of them totaling 40 students (51.3%) had normal nutritional status. The results of regression logistic test showed that body image was related to nutritional status with the p-value <0.001, whereas physical activities and nutritional status had no correlation as evidenced by the p-value <0.068. In conclusion, there is a correlation between body image and nutritional status in adolescents, whereas physical activities are not correlated with nutritional status. Hence, health education needs to be provided for the female students so that they will give more appreciation to their body image and increase thir physical activities to maintain their normal nutritional status
