Synthetic Thyrotropin-Releasing Factor, I -pyroglutamy1-1-histidy1-1-proline amide , has been administered to 145 volunteers of euthyroid and of various endocrine disorders in doses ranging from 25 to 200 pg. Thyrotropin releasing effect was assessed by serial measurement of plasma Thyroid Stimulating Hormone (TSH) using radioimmunoassay with double antibody technique, serum PBI and T3 resin sponge uptake (T3RSU) . A rise in plasma TSH was observed within five minutes after intravenous administration of 25-200 pg of TRF, reaching maximum in 15 to 30 minutes in euthyroid subjects . Graded responses were obtained as the dose levels of TRF were increased. TRF in doses of 2,000-4,000 pg also induced plasma TSH increase during 2-4 hours after oral administration. There was a rise in serum PBI in 4 out of 10 subjects while T3RSU remained unchanged. In the cases with hypothalamo-pituitary disorder including pituitary adenoma and craniopharyngioma, the TSH response to TRF in patients who showed thyroidal hypofunction was generally reduced. However, there could be observed optimal TRF induced TSH increase in hypothalamo-pituitary disorder having a euthyroid function. Almost all patients with hyperthyroidism showed no response, the plasma TSH level remaining undetectable throughout the studies, some showed a suboptimal response and a few delayed response: the plasma TSH increases only at 60 minutes. In hyperthyroidism there was no correlation between the level of serum PBI and the responsiviness to TRF. Primary hypothyroid patients have elevated plasma TSH levels at zero time and showed exaggerated response and the peak of TSH increase was somewhat delayed and continued. Longterm glucocorticoid-treated patients and two cases with Cushing's syndrome showed an inhibition of TSH response to TRF. Euthyroid subjects over 70 years old also showed optimal responses to TRF. It was concluded that administration of TRF in a single injection of 50 or 100 teg provided as a useful test of pituitary TSH reserve. (See pp. 301-,-321)
