he Japanese Assessment of Cardiac Events and Survival Study by Quantitative Gated SPECT (J-ACCESS) is the first large-scale, multicenter, observational prognostic investigation to reveal the background of major cardiac events, the prognostic value of functional and perfusion data of gated single-photon emission computed tomography (SPECT) imaging, and the 3-year event rate in 4,629 Japanese patients with known or suspected coronary artery disease (CAD).1–5 The recently published summary of the results of the J-ACCESS study demonstrated the significant predictive value of a large perfusion abnormality, decreased ejection fraction, and a large end-systolic volume assessed by ECG-gated perfusion SPECT for major cardiac events.4 Cardiac event rates in Japanese patients, however, are significantly lower (4.3%/ 3 years) than those in North American and European patients,6–13 despite the inclusion of congestive heart failure among the major cardiac events (cardiac death and nonfatal myocardial infarction (MI)) as a primary endpoint. Although age and diabetes mellitus were also significant risks for major cardiac events, the prognostic efficacies of other pre-test clinical information, such as symptoms, drugs used, and complicated non-cardiac atherosclerotic diseases, were not clarified in the previous analysis.4 The presence of diagnosed CAD per se is known to be a major clinical risk and a secondary prevention strategy is crucial for the long-term management of this high-risk population. The cardiac event rate in patients with known CAD is expected to be greater than that in patients with suspected CAD.6–13 This sub-study of J-ACCESS, therefore, focused on patients with established CAD with the aim of more precisely identifying pre-scan clinical risks and to clarify the incremental prognostic value for major cardiac events of perfusion and functional variables by stress/rest gated SPECT.
