A clearer concept of the differential diagnosis of the nephrotic syndrome in children and its prognosis in individual cases is needed. As a contribution to the natural history of this syndrome, the functional capacity of the kidneys in the course of the disease was studied by means of clearance tests. Renal clearance studies in children (1-6) and adults (7, 8) with the nephrotic syndrome have been reported in the past. The conclusions of these reports have not been uniform, probably because patients in different phases of the nephrotic syndrome were selected or because the patients were not observed over a sufficient length of time for study. In the present study the clearance tests were, wherever possible, repeated in individual patients at intervals over several years, until the outcome of the case appeared established. The results were correlated with the patients' clinical course and with other laboratory studies such as the analysis of the urine, including the Addis count, and of the chemical composition of the blood serum.
