Discussion: The correct assessment of insight is critical for clinical trials and for understanding risks to the patient in terms of relapse or nonadherence. Despite the limited sample, there appeared to be regional differences in either the assessment of item G12 by raters or the individual understanding of mental illness by patients. Brazil and Mexico had the most significant issues with either underscoring this item or misattribution of the symptom. 54% of the total potential error concentrated around item G12 and close to half of these cases identified had mild or moderately severe rater error. It is important to consider surveillance models as well as initial training around how to best assess this item of the PANSS as well as insight more generally as it relates to overall symptom severity and risk profile.
