Group reminiscence is considered as a low-cost intervention. This study aimed to investigate the effect of structured group reminiscence on depression and anxiety in elderly women who use a hookah. This is a clinical trial study. The participants of this study consisted of 24 elderly women who used the hookah. The samples were then divided into 2 groups of 12 people. Reminiscence protocol was performed in 12 sessions. The participants were asked to complete the Depression, Anxiety Scale, and Mental Screening Test in the pretest, immediately after the intervention, and one month after the intervention. The two groups were similar in terms of demographic variables ( P  &gt; .05). In the intervention group, a statistically significant difference was observed among the variables of depression, anxiety and the number of smoking daily in three stages ( P  &lt; .05). Also, comparison of the intervention and control groups showed that the differences were significant ( P  &lt; .05). Structured group reminiscence is an effective intervention used to reduce the symptoms of depression and anxiety and the number of daily smoking among the elderly women who use a hookah.
