Multiple myeloma often leads to fatigue, muscle wasting, and changes in bony structure. Current evidence is limited regarding selection of appropriate interventions to treat these impairments when a fracture is present. The purpose of this case report is to describe the safety and effectiveness of a strategic physical therapy (PT) approach to improve function in a patient with multiple myeloma recovering from a vertebral fracture. A 66-year-old male was treated for 10 weeks in an outpatient PT clinic after he sustained an L2 vertebral fracture secondary to radiation treatment for multiple myeloma. Impairments included lower extremity (LE) and core weakness, balance deficits, pain, and fatigue. PT interventions included strengthening, functional activities, and a walking program. Outcome measures included the Oswestry Disability Index (ODI), 6-Minute Walk Test (6MWT), and 5-Times Sit to Stand (FTSST). The 6MWT and FTSST scores improved at 5-week follow-up and time of discharge. Additionally, there were no adverse outcomes related to this plan of care. Objective findings suggest PT for this patient was safe and effective. Further research is needed to examine the effects of PT for patients with various locations of fractures and various stages or types of cancer.
