This article presents the model of a short-termcase management program focused on reducingemergency department presentations andunplanned hospital admissions for a targetedgroup of older people with complex care needs.As a semi-integrated health care program,Treatment Response and Assessment for AgedCare (TRAAC) is implemented by short-termcase managers located in a variety of communityagencies as well as acute and sub-acutehospital settings. The article discusses the featuresof the model including case finding, earlyintervention and risk screening, combined withthe rapid mobilisation of specialised geriatricassessment services. The model has the potentialto contribute to positive results in managingthe complex health needs of this group. Evaluationoutcomes including reductions in hospitaluse for the target group, and positive client andstaff perceptions of the service model are discussedin relation to the unique features of theintervention program.
