Methicillinresistente Staphylococcus aureus (MRSA) sind weltweit bedeutende nosokomiale Krankheitserreger, deren Haufigkeit auch in Deutschland standig zunimmt. Auf der Basis einer systematischen Literaturrecherche wurde der Evidenzgrad von Masnahmen zur Kontrolle der Ausbreitung von MRSA bewertet, der Schwerpunkt lag dabei auf der Situation in deutschen Krankenhausern. Interventionsstudien aus sechs Datenbanken (publiziert von 1980 bis November 2001) gingen in die Recherche ein. Zudem berucksichtigt die Analyse Ergebnisse von Referenzlisten, manueller Suche, Literatursammlungen und auch Expertenmeinungen. Zielgrosen waren die Kolonisation oder nosokomiale Infektionen von Patienten und/oder Krankenhauspersonal mit MRSA. Nur wenige Masnahmen fur die Pravention und Kontrolle von MRSA sind evidenzbasiert und bezuglich ihrer Kosteneffektivitat unter verschiedenen medizinischen Bedingungen untersucht worden. Dies gilt vor allem fur das Screening von Risiko-patienten. Empfohlen wird, MRSA-Patienten zu isolieren; von besonderer Bedeutung ist die Handehygiene und auch eine kontrollierte und optimierte Anwendung von Antibiotika. The evidence on the effects of interventions to control the spread of MRSA was systematically reviewed, whereby interest focused in particular on the current situation of hospital care in Germany. Studies dating from 1980 to November 2001, regardless of format, identified in six medical databases, reference lists and personal collections were included. The main outcome measure was colonisation or nosocomial infection of patients and hospital staff with MRSA. Few studies provided economic data, and the majority of the articles were expert judgements, consensus statements, or reports. Few hygiene measures have proven to be evidence-based and appropriate to control the spread of MRSA, or have proven to be cost-effective in various acute-care settings (especially screening of patients at risk). Isolation of MRSA patients is generally recommended, and improving hand hygiene is of high importance.
