Screening studies on the amount of thrombin bound to intact human platelets show that deficiencies are present in certain disorders which are associated with hypercoagulable states. Among the hematologic diseases where this is found most prominantly are primary thrombocythemia and polycythemia vera. A direct relationship can be plotted between the decrease in thrombin binding and the proportion of abnormal platelets present in the circulation. A deficiency also is present as an acquired disorder in occasional patients with advanced cancer in whom spontaneous thrombotic events complicate the clinical course. These include adenocarcinomas of the breast, colon and lung. It does not appear to be present in patients with epidermoid cancers. Whether these platelet changes bear a cause and effect relationship between growth and dissemination of the specific neoplasms or are merely a consequence of a generalized hypercoagulable state is unknown. However, the conditions in which the thrombin binding potential of the platelets is decreased coincide closely with separately published findings in which the thrombin binding potential of adenocarcinoma cells was shown to be different from epidermoid carcinoma and comparable normal control tissues.
