It is well known that ethylestrenol and/or phenformin can normalise deficient fibrinolysis in the vessel walls and prevent recurrency of thromboembolism (Nilsson et al 1975, Hedner et al 1976). Because of the side-effects of these drugs we have studied the effect of a phenformin- like substance: moroxydinechloride (Kabi 1886) which does not cause lactic acidosis. Sixty-four patients with decreased fibrinolytic activity in vessel walls (histo- chemical assay of superficial hand vein according to Pan- dolfi et al 1971) and/or decreased release mechanism (venous occlusion test for 20 min as described by Robertson et al 1972) on repeated investigations are taking part in a prospective randomised clinical trial in which they receive Kabi 1886 in a dose of 0.04 g/kg and day or no specific treatment. All of these patients have earlier had one or more deep venous or arterial thrombosis or superficial thrombophlebitis. At check up 6 months after entering the trial it was found that 15 of 23 patients (65 %) with deficient release mechanism were normalised. Spontaneous normalisation in the control group was found in 5 of 18 patients (28 %). Some of the patients have been investigated also after 12 months and the tendency is the same as after 6 months. Treatment with Kabi 1886 thus seems to be effective in a fairly high percentage of patients with deficient release mechanism after venous occlusion.
