A sindrome confusional aguda (SCA) ou delirium e uma complicacao comum em pacientes submetidos a cirurgia, em especial as cardiacas e neurologicas, sendo uma alteracao cognitiva. Se nao abordada imediata e corretamente, pode provocar sequelas irreversiveis. E uma situacao clinica ainda pouco estudada quando tratada conjuntamente com a neurocirurgia, necessitando de estudos prospectivos mais adequados. Esta revisao tem como proposito elucidar questoes referentes a uma complicacao neurocirurgica subdiagnosticada, que em muitos casos nao recebem o tratamento mais indicado pela literatura. Alem de abordar a etiologia, o diagnostico e o tratamento dessa complicacao, o presente trabalho enaltece a necessidade de mais estudos acerca da SCA e suas derivacoes, tendo, assim, o proposito de promover a melhor abordagem pos-operatoria de uma das complicacoes mais complexas da neurocirurgia.(AU) Acute confusional syndrome (ACS) or delirium is an usual complication in patients submitted to surgery, especially heart and neurological, result of a cognitive disturb. If not dealt immediate and properly, it may cause irreversible consequences. It is yet a poorly studied clinical condition, when treated along with neurosurgery, more appropriate prospective studies being necessary. This review/paper aims to elucidate issues related to an underdiagnosed neurosurgical complication which, in many cases, do not receive the best treatment indicated by literature/textbooks. Besides approaching the etiology, diagnosis and treatment for this complication, the current work/paper emphasizes the need for more studies about the ACS and its derivatives. Thereby, it features the purpose of promoting the best postoperative approach for one of the most complex neurosurgery complications(AU)
