La metformine est un biguanide hypoglycémiant largement utilisé. Bien que sa marge thérapeutique soit grande, une acidose lactique mortelle peut survenir rapidement en cas de surdosage massif. Le diagnostic différentiel de l'acidose est facilité par le dosage de la metformine plasmatique. L'objectif de ce travail est de mettre au point et valider une méthode de dosage ultra-rapide de la metformine, petite molécule très polaire difficile à extraire par les solvants classiques et difficile à doser en CLHP en phase inverse. La méthode choisie est la chromatographie d'interaction hydrophile (HydrophILic Interaction Chromatography, HILIC) avec une détection par barrette de photodiodes. La colonne contient une phase stationnaire particulière, la poly(2-hydroxyéthylaspartamide)-silice (colonne PolyHydroxyEthyl A). Elle est thermostatée à 50° C. La phase mobile est un mélange acétonitrile/acide phosphorique 65 : 35, v/v, à pH 2,8. Le débit est de 1,5 ml/min. Dans ces conditions, le temps de rétention de la metformine est de 2,9 min. Elle est isolée à partir de 250 µl de plasma déprotéinisé par 15 µl d'acide perchlorique. Le dosage est réalisé par rapport à une courbe d'étalonnage. Dans ces conditions, le temps de rétention de la metformine est de 2,9 min. La méthode est linéaire de 0,1 à 400 µg/ml. La limite de détection est 0,02 µg/ml, la limite de quantification de 0,1 µg/ml. Les variabilités sont plus importantes aux faibles concentrations qu 'aux fortes concentrations. La récupération de la metformine plasmatique est quasiment totale (rendement de 99,5 %). Les autres antidiabétiques oraux ne donnent pas d'interférence. L'application à un cas de suicide par ingestion massive de Glucophage® montre une concentration plasmatique de 188 µg/ml, en accord avec l'hyperacidose lactique retrouvée pendant la tentative de réanimation.Metformin is a biguanid oral hypoglycemic agent, widely used in France. The aim of this work is to develop an ultra-short technique for the determination of metformin in plasma. This little molecule is a very polar one and is very difficult to extract by classical solvents and to assay by reverse phase HPLC. That is why we choose hydrophilic interaction chromatography (HILIC), with a PolyHydroxyEthyl stationary phase. Mobile phase is acetonitrile : phosphoric acid (65 : 35, v/v) at pH 2,8.Chromatography is achieved at 50° C, with a flow-rate at 1.5 ml/min. In these conditions the mean retention time of metformin is 2.9 min. It is isolated from 250 µl plasma by precipiting the proteins by 15 µl perchloric acid. The mean retention time is 2.9 min. The linearity of the method is very good from 0.1 to 400 µg/ml. The limit of detection is 0.02 µg/ml, the limit of quantification is 0.1 µg/ml. Intra-day and inter-day variabilities are higher at low concentrations than at high concentrations. Recovery is excellent, reaching 99.5 %. We verified that there is no analytical interference between metformin and other oral hypoglycemic agents. Using this new ultra-short method, we report a case of a suicide attempt with Glucophage®, with a severe lactic acidosis and a plasma metformin concentration of 188 µg /ml. The patient died 34 hours after ingestion
