ResumenPropósitoEl propósito del presente trabajo es evaluar el éxito de un protocolo de carga inmediata utilizando implantes zigomáticos como anclaje remoto, en combinación con 4 implantes convencionales en la región maxilar anterior, en el tratamiento protésico de los maxilares severamente reabsorbidos.Material y métodosEste estudio clínico incluyó un total de 48 implantes de carga inmediata distribuidos en 16 implantes zigomáticos bilaterales (Branemark Nobel Biocare), en combinación con 32 implantes convencionales (Replace Select Tapered de Nobel Biocare y SFB de Alpha Bio TEC) en la región anterior y les fueron instaladas prótesis híbridas atornilladas metal / resina de arco cruzado, sin extensiones de cantilevers, 24 a 48 hrs. después de la cirugía de implantes, en un total de 8 pacientes desdentados completos maxilares, 5 mujeres y 3 hombres con un promedio de edad de 52.5 años, que cumplían con los criterios de inclusión.ResultadosTodos los pacientes seleccionados para este trabajo clínico fueron operados satisfactoriamente y no presentaron complicaciones quirúrgicas durante la inserción implantaria, obteniendo un promedio de estabilidad implantaria (ISQ) medido con el instrumento Ostell Mentor de 72.3 para los implantes zigomáticos y un torque quirúrgico de inserción promedio para los implantes convencionales de 46.4 Ncm. La tasa de éxito para los implantes zigomáticos e implantes convencionales, así como para el tratamiento protésico, fue del 100% después de 18 meses de carga funcional.ConclusionesCon las limitaciones de este preliminar estudio, la indicación e instalación de implantes zigomáticos para el tratamiento de los maxilares severamente reabsorbidos, bajo la modalidad protésica de Carga Inmediata, es una alternativa altamente eficiente, sin largos tiempos de espera para los pacientes (reporte de casos).AbstractPurposeThe purpose of this study was to evaluate the success of Immediate Loading protocol using Zygomatic Implants as remote anchors in combination with 4 conventional implants in the anterior maxillae region in the prosthetic treatment of severely resorbed maxillae.Material y MethodsThis clinical study included a total of 48 immediately loaded implants distributed among 16 bilateral zygomatic implants (Branemark Nobel Biocare) in combination with 32 conventional implants (Replace Select Tapered Nobel Biocare and SFB Alphabio TEC) in the anterior region maxillae. Hybrid prostheses were installed with metal screws/ cross arch resin without cantilevers extensions 24 to 48 hrs. after implant surgery, in a total of 8 patients with completely edentulous maxillae, 5 women and 3 men with a mean age of 52.5 years, who met the inclusion criteriaResultsAll patients selected for this clinical work were operated on successfully without surgical complications during implant insertion, obtaining an average of implant stability (ISQ), measured with the Ostell Mentor instrument, of 72.3 for zygomatic implants and an average surgical insertion torque for the conventional implants of 46.4Ncm. The success rate for zygomatic implants and conventional implants, as well as for the prosthesis treatment was 100% after 18 months of functional loading.ConclusionsWith the limitations of this preliminary study, the indication and installation of zygomatic implants for the treatment of severely resorbed maxillae, under the form of Immediate occlusal function is a highly efficient alternative, without long waiting times for patients (reported cases)
