ResumenIntroducciónLa seudoartrosis es la detención en el proceso de consolidación de una fractura ósea, justificada por la ausencia de signos clínicos y radiográficos que así lo confirmen, en un período de tiempo máximo establecido (9 meses) y que precisa de una intervención quirúrgica para su resolución.En el caso de una seudoartrosis séptica, los microorganismos implicados en esta enfermedad se benefician de la pérdida de hematoma fracturario y colonizan dicho foco, generando un biofilm que los protege. Dentro de estos, el más común es el Staphylococcus aureus.ObjetivoEvaluar el tratamiento de la seudoartrosis séptica de tibia mediante el uso de clavo con cemento con antibiótico.Materiales y métodosSe evaluó a 4 pacientes con fracturas abiertas de tibia, entre los años 2007 y 2013, los cuales recibieron tratamientos iniciales distintos para resolver sus fracturas y que concluyeron con seudoartrosis séptica en todos los casos. Los 4 pacientes fueron reintervenidos a fin de eliminar el foco infeccioso y en el mismo tiempo se les colocó clavo de cemento preparado con antibióticos.ResultadosLos pacientes negativizaron los cultivos entre el 3.er y 13.er mes de colocado el clavo con cemento con antibiótico. En todos los casos, los elementos de laboratorio monitorizados comenzaron a descender a la semana de la colocación del clavo. La consolidación definitiva de las fracturas solo se logró en un caso, mientras que en el resto se optó, a los 6 meses de no encontrar evidencia radiográfica de consolidación, por realizar una nueva cirugía de estabilización. Finalmente se logró la consolidación entre los 4 y 6 meses posteriores a la última cirugía.DiscusiónEn este trabajo hemos encontrado que el uso del clavo con cemento con antibiótico es una elección más que válida para el tratamiento de la seudoartrosis séptica de tibia, no solo por lograr la resolución de la infección, sino, además, por favorecer la consolidación ósea subsiguiente.ConclusiónCreemos que el clavo endomedular con cemento con antibióticos, previo fresado del canal medular, es una técnica con resultados alentadores para el tratamiento en la seudoartrosis séptica de tibia. The definitive consolidation of fractures was only achieved in one case, while the rest were reoperated after 6 months of absence of radiographic evidence of consolidation. Consolidation between 4 and 6 months after the last surgery was finally achieved.DiscussionIn this work, we have found that the nail with cement with antibiotic is a valid option for the treatment of septic nonunion of tibia, not only to achieve resolution of the infection, but it is also improving the subsequent bone consolidation.ConclusionWe believe that the endomedular nail of cement with antibiotics, prior reaming of the medullar canal, is a technique with encouraging results for the treatment in septic nonunion of tibia. It provides relative fracture stability, higher simplicity in handling soft parts and consolidation facilities.Evidence levelI
