ResumenIntroducciónLa hernia de Spiegel supone alrededor del 1% de todas las hernias de pared abdominal. La presencia de una apendicitis por incarceración apendicular en una hernia de Spiegel es un caso excepcional.Caso clínicoSe presenta el caso de una mujer que consultó por dolor abdominal y tumoración abdominal irreductible en el flanco izquierdo. El tac mostraba una hernia de Spiegel incarcerada con presencia del apéndice en su interior, que se confirmó en una intervención quirúrgica urgente.DiscusiónLa hernia de Spiegel es un defecto de pared abdominal infrecuente, que representa del 0.1% al 2% de todas las hernias. Se define como la protrusión de un saco peritoneal, órgano o grasa preperitoneal que se presenta en la aponeurosis Spiegel, en situación lateral a la línea semilunar, habitualmente por debajo del arco de Douglas. El contenido herniario suele ser epiplón, intestino delgado, colon y, en raras ocasiones–como en el caso que se describe–, el apéndice cecal. El tratamiento debe ser siempre quirúrgico, y en caso de complicación es necesario el tratamiento urgente.ConclusionesLa incarceración de una hernia de Spiegel que se presenta junto con una apendicitis aguda es un proceso poco frecuente que se ha descrito en pocas ocasiones en la bibliografía médica y que precisa un alto nivel de sospecha clínica para realizar un diagnóstico y tratamiento correctos.AbstractIntroductionSpigelian hernias account for 1% of all abdominal wall hernias. Exceptionally, a Spigelian hernia may present appendicitis due to an incarcerated appendix.Clinical caseWe present the case of a female reporting abdominal pain and an irreducible mass on the left abdominal flank. The computed tomography scan revealed an incarcerated Spigelian hernia with the presence of appendicitis, thus confirming the need for an emergency surgery.DiscussionSpigelian hernia is an uncommon abdominal wall defect, representing between 0,1 and 2% of all hernias. It is defined as the protusion of a peritoneal sac, organ or preperitoneal fat presented in Spigelian aponeurosis, lateral to the linea semilunaris, usually below the Douglas line. Hernia contents usually comprise the omentum, small intestine, colon and, rarely—such as this particular case—, the cecal appendix. Treatment must always be surgical, being urgent treatment necessary should any complications arise.ConclussionsIncarcerated Spigelian hernias associated with acute appendicitis are an uncommon condition that has not been described frequently in the literature and that requires a high level of clinical suspicion in order to reach an accurate diagnosis and treatment
