Background The recent multicenter, randomized, openlabel\udINDEPENDENT study demonstrated that sevelamer\udimproves survival in new to hemodialysis (HD) patients\udcompared with calcium carbonate. The objective of this\udstudy was to determine the cost-effectiveness of sevelamer\udversus calcium carbonate for patients new to HD, using\udpatient-level data from the INDEPENDENT study.\udStudy design Cost-effectiveness analysis.\udSetting and population Adult patients new to HD in Italy.\udModel, perspective, timeframe A patient-level cost-effectiveness\udanalysis was conducted from the perspective of the\udServizio Sanitario Nazionale, Italy’s national health service. The analysis was conducted for a 3-year time horizon. The\udcost of dialysis was excluded from the base case analysis.\udIntervention Sevelamer was compared to calcium\udcarbonate.\udOutcomes Total life years (LYs), total costs, and the\udincremental cost per LY gained were calculated. Bootstrapping\udwas used to estimate confidence intervals around\udLYs, costs, and cost-effectiveness and to calculate the costeffectiveness\udacceptability curve.\udResults Sevelamer was associated with a gain of 0.26 in\udLYs compared to calcium carbonate, over the 3-year time\udhorizon. Total drug costs were €3,282 higher for sevelamer\udversus calcium carbonate, while total hospitalization costs\udwere €2,020 lower for sevelamer versus calcium carbonate.\udThe total incremental cost of sevelamer versus calcium\udcarbonate was €1,262, resulting in a cost per LY gained of\ud€4,897. The bootstrap analysis demonstrated that sevelamer\udwas cost effective compared with calcium carbonate\udin 99.4 % of 10,000 bootstrap replicates, assuming a\udwillingness-to-pay threshold of €20,000 per LY gained
