which service provider or providers should receive the funds to deliver the services. Communities may decide that their own organisation should provide all primary care, or that the State health department should accept the responsibility. Alternatively, they may decide that their own organisation should provide some aspects of primary care, and the State, and possibly others, should receive funds for other aspects. The Commonwealth would then distribute funds accordingly. Communities could review their services every two or three years and, if they were dissatisfied with the services, alternative arrangements could be made. This proposal would reduce the current funding complexity and would be consistent with the principle of self-determination.
