The management of Bell's palsy can be summarized as follows. 1. A clear distinction must be made between upper and lower motor neuron paralysis. 2. The site of the lesion must be determined according to Tschiassny's criteria. 3. The cause of the lesion must be determined, with Cawthorne's famous dictum in mind — “All that palsy's is not Bell's”. 4. By means of modern electrical tests, the presence of axonal degeneration must be determined. ACTH gel or corticotropin is injected daily according to Tavener's dosage scheme. If axonal degeneration takes place despite the administration of ACTH gel, decompression of the nerve is indicated.
