Summary We discuss the case of a female with complete sacral agenesis scheduled for an elective caesarean section. Patients with this condition present a challenge to the anaesthetist, and usually receive a general anaesthetic, due to concerns regarding the precise underlying anatomy and the unpredictability of local anaesthetic spread. We believe that this is the only reported case of a patient with complete sacral agenesis undergoing spinal anaesthesia for an elective caesarean section.
