filled with two layers of mucoperichondrium (d). Some would advocate a three layered closure (mucoperichondrium ⁄ cartilage ⁄ mucoperichondrium) being necessary for closure to limit recurrence of the perforation. We know from our experience with septoplasty that it is common practice to remove quadrilateral cartilage leaving two layers of mucoperichondrium without significant risk of perforation. We have had excellent results with this technique and advocate it as we believe it is safe, relatively simple, low risk, time efficient and inexpensive. Conflict of interest
