A complicated periapical infection of a right lateral maxillary incisor is reported. Conservative treatment including standardized mechanical preparation and local medication with calcium hydroxide was not successful. Neither was directed antibiotic therapy effective in eliminating the infection. Periapical surgery was performed and the healing proceeded uneventfully. Bacteria were isolated both from the root canal and the periapical lesion. The findings are discussed in relation to phagocytosis and bacterial synergism.
