Numerous sudanophilic myeloblasts were found in the bone marrow and peripheral blood of a patient with chronic myeloid leukaemia who developed acute blastic transformation. The majority of blasts contained numerous large lipid‐containing vacuoles in their cytoplasm which stained positively with Sudan Black and were obvious on electronmicroscopy. The rarity of this unusual finding is stressed.
