Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) syndrome is an extremely rare diagnosis in elderly patients. We describe a 73‐year‐old female with ALCAPA who underwent successful repair of this coronary anomaly. (J Card Surg 2011;26:66‐68)
