Pectoral ICD implantation, although feasible with the release of smaller devices, can be cosmetically disturbing to some patients due to the device protruding under the skin. An ICD was implanted using the retromammary approach in a 25‐year‐old female patient. Retromammary implantation of an ICD is feasible and offers an alternative approach in women.
