Summary: Complete embedding or incarceration of the IUD is likely to cause difficulty at removal. Attempts at vaginal removal with ‘heavy’ instrumentation are hazardous and should be abandoned in favour of an abdominal approach. A trial demonstrating the superiority of the latter approach is described. The characteristics of patients with an incarcerated loop make concomitant tubal ligation possible in most instances.
