A Harmless Method for the Assessment of the Patency of Ventriculoatrial Shunts in Hydrocephalus

SUMMARY A simple, harmless method was devised to determine the patency of ventriculo‐atrial shunts, based on the principle that a temperature variation introduced at one point on a tube containing flowing liquid would be carried along with the movement of the liquid and could be recorded at a later point in the flow. The equipment, consisting of a thermosensitive device, a cube of ice and a Wheatstonebridge system connected to a recorder, was used to test the patency of a variety of shunts. Results of the measurements, compared with findings at operation and with the clinical condition, were accurate to the order of 90 per cent in a total of 67 examinations. RÉSUMÉ Une méthode simple et sans danger a été mise au point pour determiner l'ouverture des shunts ventriculo‐atrial; elle est basée sur le principe qu'une variation de température introduite à un point d'un tube contenant un liquide qui s'écoule serait transportée avec le mouvement du liquide et pourrait être enregistrée plus loin dans le courant. L'équipement, qui consiste en un appareil thermosensitif, un cube de glace et un système ‘Wheatstone‐bridge’ connectéà un appareil d'enregistrement, a été utilisé pour tester l'ouverture d'une variété de shunts. Les résultats des mesures, comparés avec les données de l'opération et avec l'état clinique, étaient exacts jusqu' à 90 pour cent. ZUSAMMENFASSUNG Eine einfache, harmlose Méthode, die auf dem Prinzip beruht, dass eine Temperatur‐Variation, die an einem Punkt an einer flüssigkeitsenthaltenden Röhre eingeführt, mit der Flüssigkeitsbewegung entlang getragen wird und an einem späteren Punkt in der Flüssigkeitsbewegung verzeichnet werden kann, ist entworfen worden um das Offenstehen der Ventrikulo‐Atrial‐Shunts festzustellen. Die Ausstattung, die aus einem thermosensitiven Gerät, einem Würfel Eis und einem an einen Registrierapparat angeschlossenen Wheatstone‐Brückensystem besteht, wurde benützt, um an verschiedenartigen Shunts zu prüfen, ob sie offenstanden. Die Messungsresultate die mit den Operationsbefunden und dem klinischen Zustand verglichen wurden, waren in 90 prozent der Fälle zutreffend. RESUMEN Se ha inventado un método sencillo y sin peligro para determinar la patencia de shunts ventrículo‐atriales, el cual se basa en el principio que una variación de temperatura introducida a un punto fijo en un tubo que contiene líquido fluyente será transportada con el movimiento del líquido y podrá ser registrada a un punto más adelante en el flujo. Se ha empleado el aparato, que comprende un mecanismo termosensible, un cubo de hielo, y un sistema Wheatstone‐bridge acoplado a un registrador, para valorar la patencia deu na variedad de shunts. Los resultados de estas mediciones, comparados con los hallazgos operatorios y el estado clínico, eran exactos en un 90 por ciento de los casos.
