We present the case of a 26‐year‐old Caucasian male with an ulcerative necrobiosis lipoidica (NL) affecting approximately 30% of the skin including the scalp and face. The disease started at (he age of six and has progressed constantly since then. Over the last 20 years no diabetes mellitus or diabetic disposition was found. Up to now all treatments, e.g. oral corticosteroids and pentoxifylline have failed to stop the progression of the disease. To the best of our knowledge this is the most extensive case of NL described so far.
