Isolated chylomediastlnum developed In a patient after mitral valve replacement for rheumatic mitral stenosis. The site of injury to the thoracic duct was shown by lymphangiography to be at the level of the sixth thoracic vertebra. After conservative measures had failed, surgical ligation of the thoracic duct resulted in immediate cure.
