SUMMARY Painful tonic seizure (PTS) was observed in four patients with multiple sclerosis. Various kinds of drugs were ineffective, but diphenylhydantoin and/or carbamazepine were proved to be useful in suppressing the seizure completely. To our knowledge, this is the first report concerning the satisfactory effect of carbamazepine on the PTS in multiple sclerosis. From the clinical informations observed in our cases including H‐reflex study, we postulated that the painful tonic seizure might be due to an unusual excitation of a sensory‐motor neuronal complex and unusual transmission or conduction of the impulses in the affected spinal cord level or in the lower brain stem.
