An ossifying malignant Leydig cell tumour of the testis in a patient with associated gynaecomastia is presented. The criteria for diagnosing malignancy in these tumours are briefly discussed. After left orchidectomy he received cyclical chemotherapy consisting of bleomycin, etoposide and cis‐platinum. At three months the gynaecomastia and pulmonary lesions have regressed and serum oestrogen has gradually fallen to 228 i.u./l.
