S.McK., male, aged 82. Hospital no. 379057. History. This retired seaman presented in July 1981 with a 2-month history of increasing swelling and redness of the nose. There was no nasal obstruction or epistaxis. His general health had been good. Examination. There was diffuse swelling and discolouration of the nose but no papules or pustules (Fig. t). The right tonsil was moderately enlarged. Small firm lymph nodes were palpable in both axillae and hepatosplenomegaly was just detectable. Investigations. Haemoglobin 154 g/dl. Platelets 255 x io'/l. White bkwd count 36-8 x io'/l with 80",, lymphocytes. Blood film showed smear cells and primitive lymphoid cells. Bone marrow showed increased ceilularity with predominance of lymphoid cells, some with primitive characteristics. Chest X-ray and liver and spleen scans were normal. Histology. Biopsy of the nose showed a dense monomorphous infiltrate with lymphoid cells penetrating deep into the dermis to the full depth of the biopsy.
