This is a case of a 74‐year‐old male with known multiple peripheral aneurysms. The patient presented for resection of a right popliteal aneurysm. During a routine preoperative transthoracic echocardiography, we detected a cystic structure on the inferolateral aspect of the basal left ventricle and the left atrium along the atrioventricular groove. A CT scan confirmed this to be a left circumflex artery aneurysm. The patient subsequently died of a ruptured berry aneurysm and an intracranial bleed. (ECHOCARDIOGRAPHY, Volume 21, April 2004)
