Blumberget al.doi.10.1093/eurjhf/hft0Pulmonary arterial hypertension (PAH) and chronic thrombo-embolicpulmonaryhypertension(CTEPH)arerareconditionsasso-ciatedwithapoorprognosis,forwhichtreatmentsareexpensiveand,at best, only moderately efcacious. Until the very latest stages ofdiseaseprogression,symptomsoccuralmostexclusivelywithphysic-al exertion, most probably reecting insufcient oxygen delivery toperipheral muscles caused through limitations of right ventricular(RV) reserve relative to heightened pulmonary vascular afterload.
