Offshore pipeline is an indispensable component that plays a huge role in the distribution of hydrocarbon products. Thus, it is necessary to prevent any kind of incidents that may bring harm to the offshore pipeline system. Therefore, this paper is intended to develop the structure of Safety Case Document which provides preventions and mitigations actions and formulate risk management approach for offshore pipeline. Offshore pipeline that was specifically being used as a data in this project is located at Northeast of Java Sea and possesses 1 km of length. This pipeline has sustain from pipeline leakage, therefore this paper will identify the risk assessment for pipeline damage.. The methods that was being involved in this project are as follows; HAZID to identify the cause of the failure of the pipeline and Fuzzy FMEA to determine the Risk Priority Number in order to find out the priority of each failure cause. HAZID also useful to provide the necessary prevention and mitigation actions in accordance to each cause and subsequently being structured as a Safety Case Document.
