Summary We describe the problems encountered during general anaesthesia in a 2‐year‐old boy who was suffering from an extremely severe form of Niemann‐Pick disease. Attention focused on respiratory difficulties and intubation problems. The usefulness of the lateral position and the limits of the Mallampati score in this case are discussed.
