3T MR in 47 asymptomatic subjects at risk for C9ORF72 mutation. 3DT1 MRI sequences were analyzed using Freesurfer to extract the volumes of subcortical structures in each subject. Volumes were compared between carriers and non-carriers using multiple t-tests. In carriers and non-carriers, linear regression with age and correlation coefficient with age were calculated for each structure; slopes of linear regression were compared between carriers and non-carriers using ANCOVA. Correction for multiple comparisons was performed using the Holm-Sidak method (alpha 1⁄4.05). Results:Among the 47 subjects, 25 carried the C9ORF72 mutation and 22 were non-carriers. There was no significant difference between the 2 groups regarding age & gender. Mutation carriers showed statistically significant atrophy in the left cerebellum cortex, as compared to non-carriers (p 1⁄4.0095, corrected). Among the 22 subcortical structures analyzed, 9 had a volume significantly correlated with age in mutation carriers (p<.05, corrected), while no correlation remained significant in non-carriers after correction for multiple comparison. When comparing the slope of linear regression with age of subcortical structure volumes, slopes were significantly lower in carriers for left & right putamen (p1⁄4.02 & .01, respectively), left pallidum (p1⁄4.03) and left accumbens (p1⁄4.02). Conclusions: Compared to non-carriers, presymptomatic C9ORF72 carriers demonstrate cerebellar atrophy and accelerated age-related atrophy of subcortical structures. These findings could be used in future disease-modifying therapies in presymptomatic C9ORF72 carriers.
